About this Questionnaire.

You'll find there's a considerable amount of work in preparing your company's materials for the designs Leland will construct for you. The following questionnaire will help him define exactly what you need from your designs, and will allow him to prepare a detailed Project Proposal and/or Project Brief for you.

Please try to fill in as much information ahead of time as possible, but don't worry about being able to answer every question completely. Only those items marked with a star are required.

Leland will use these worksheets as a guide to helping develop your designs together. These worksheets are ultimately designed to enhance communication between Leland and his clients.

After it is all filled out, Click the button at the end of the form. This will send the form to Leland, providing all the details needed to provide an accurate Project Proposal/Brief.
